Where is the header from revolution slider with video?

    On your Mentor theme which we bought, there is no demo data for the second slider - with a you tube video. Same as in the screenshot. 

    How do I create or import that exact slider please. I have already imported the slider demo zip file,- but it does not show the options and only has one slide?

    Please advise :)
