I have not explained well. My English is not good. In the case of "New customer" if you send it as a hidden field. But, if it comes after logout "Current customer" as the hidden field is blank title and is not sent.
Is it possible that there is a error in the file /booked/js/functions.js? on line 602 where says: surnameActive = $ ( '# newAppointmentForm input [name = booked_appt_name]'). length, I should say: surnameActive = $ ( '# newAppointmentForm input [name = booked_appt_surname]'). length,
By selecting "Require" Name "only ...." in the configuration register leaves.

you can try deactivate then delete "booked appointments" plugin then go to Apprearance/install plugin and install "booked appointments" plugin
Hi, now Booked plugin has some issues, so you should require "First Name" and "Last Name"
